GRAMIN ACCIDENT POLICY (GAP)
Applicability:
Policy covers death or
permanent total disablement due to accident.
Age:
Person in the age
group of 10 to 70 years can be covered.
Sum Insured:

00,000/- and in extraordinary cases even beyond 5 lakh.
Benefits:

COMPREHENSIVE INSURANCE FOR IRRIGATION SYSYTEM (LIFT, DRIP, SPRINKLER ETC).
Scope of cover:
The policy provides
indemnity against damage caused to irrigation system (lift, Drip, Sprinkler etc.). The
policy covers each item of the irrigation system and its accessories.
Premium Rate:
1% of Sum Insured.
Sum Insured:
Sum Insured should be
equal to the cost of replacement of insured property by new property of the same kind and
capacity. It should include freight, custom duties and erection costs, also. Sum insured
for each part/item to be declared separately.
Risks covered:
Fire, Riot, Strike,
Malicious damage, Flood, Storm, Cyclone, Earthquake, landslide, Bursting of Pipelines,
Accidental damage to Machinery and Pipelines, Mechanical and electrical breakdown, theft.
Exclusion:
War and nuclear
perils, overload experiments/test, gradual wear and tear and deterioration, wilfull
negligence, consequential loss, terrorism, loss/damage for which manufacturer is liable,
pre-existing flaws/defects.
Claims:
In the event of claim,
from the first day of loss the sum insured is reduced by the amount of claim. In order to
prevent under insurance during the remaining period of policy, it is necessary to
reinstate the sum insured by payment of pro-rata additional premium.
An amount of Rs.1000/-
is deducted on each and every claim under policy. However, the amount of deductible, in
case of bread down claims shall be limited to Rs.250/- per claim.
COMPOSITE PACKAGE POLICY FOR TRIBALS
The policy
is designed, at a very low cost, to provide protection to Tribals in respect of assists as

It is possible to
cover Tribals on group basis, which can attract discount in premium ranging from 2.5% to
45% depending upon number of persons.
The policy is governed
by following two important provisos:
- Satisfactory health statement before
commencement of insurance
- Expenses for disease/illness contracted
by insured person during first 30 days from commencement of policy are not payable.
General:
It is possible to
issue a package policy for Farmers covering the assets and family members on similar lines
like the package cover available for Tribals.

Sum, Insured is cost of
cultivation or input cost i.e. cost of raising/development of insured trees. The policy is
for a fixed sum insured, which is given separately under each crop. Depending upon the
need of the client. Modified input clause can be incorporated. Indemnity: The policy is a
predetermined value policy. For calculating the amount of indemnity sliding scale of input
is applied for each type of crop. Franchise: No claim shall be payable if amount of claim
assessment does not exceed 10% of SI per acre or Rs. 1000/- per affected acre. Excess: 20%
of the claim assessed per acre. Losses due to any excluded perils are quantified and
deducted. |

CLAIM PROCEDURE FOR RURAL INSURANCE POLICIES
General
The general claim
procedure to be followed in case of death of an insured animal is as under:
- Immediate intimation to the insurer.
- Duly completed claimform
- Death certificate
obtained from qualified veterinarian in Company’s form
- Post-mortem examination report of
required by the Company
- Ear tag to be submitted to the company
wherever applicable.
- Photograph of dead animal wherever
necessary.
In case of scheme animals
Duly completed claim form along with
ear tag.
Certificate of death from Veterinary
Surgeon or a certificate jointly by any two of the following
- Sarpanch of the village
- President or any officer of
Co-operative Society
- Official of milk collection centre
- DRDA authorised nominee
- Supervisor/Inspector/officer of any
Banking or Credit
- Secretary or Vice President of
Panchayat.
- Village Revenue Officer/Village
Accountant
- Head master of a Primary School
- Any of the above officials have to
declare that they have seen the carcase and ear tag
intact in the ear mentioning number thereof.
- Post-mortem report, if conducted
Claim Procedure for Permanent Total
Disablement (PTD) Claims for Animals
A Certificate from the qualified
veterinarian to be obtained.
The animal should be
inspected by Company’s Vet. Officer also
Complete chart of treatment, medicines
used, receipts etc. To be provided to the
insurer
Admissibility of claim is considered
after two months of issue of Vet, Certificate Company Vet. Doctor will inspect and confirm
the disability.
Poultry Insurance
In addition to the
general claim procedure
- In case of epidemic
disease outbreak immediate notice within 12 hours to insurer and
affected birds to be segregated and produced to insurer’s
representative for inspection.
- Post-mortem report of sample birds
- Daily records of mortality, feeding
etc.
- Purchase invoices for the birds
- Any other proofs to substantiate the
loss like photographs, medical bills, etc. as and when
required.
- Daily mortality details should be sent
to the company on weekly basis, failing which report will be treated, as nil for that
particular week.
Brackish Water/Inland Fisheries Insurance
- Immediate notice within 24 hours of
loss
- Duly completed claim form
- Certificate from F.E.O (Fisheries
Extension Officer) of F,F.D.A
(Fisheries Foremen
Development Agency) or an officer of equal cadre in State Government Fisheries Department
of CIFRI (Central Inland Fisheries Research Institute) or Senior Fisheries Expert of MPEDA
(Marine Products Export Development Agency)
Certifying the cause
of death together with value of stock at the time of death with details of salvage, if
any.
Sericulture Insurance
- Immediate intimation of loss of the
Company.
- Completed claim form
- Mortality certificate by Sericulture
officer (Tech) not below the rank of Sr. Inspector of the Department of Sericulture giving
brief details of cause and extent of loss.
Honeybee Insurance
Mortality certificate
to be issued by an authorised Agriculturist.
Agriculture Pumpsets/Lift Irrigation/Gobar Gas/Hut/Animal Driven Cart Insurance
- Immediate intimation of loss to the
company
- A competent Surveyor is appointed by
the Company to conduct survey
- Police report in case of Burglary
- Claim form, final bills and receipts
Package Policy for Tribals/ Gramin Personal Accident Policy
- Immediate notice of loss to the
Company.
- Claim form
- Medical certificate and bills in case
of Permanent partial/total disablement
- In addition, for death claim nominee
should submit:
- Death certificate from competent
authority
- Original policy
